Megelőzésükre szervezetileg kiváló lehetőség a gondozásba vételkor a dohányzói status védőnői felmérése, majd a háziorvosokkal együtt a várandósok támogatása a dohányzás szünetelése vagy a leszokás érdekében. Célkitűzés: Egy mintaszerű rendszer felépítésére Nyíregyházán modellkísérletet végeztünk a városi védőnői hálózatban 2019. október 1. és 2021. március 31. között, gondozásra jelentkezett várandósok körében. Módszer: Papíralapú kérdőíveink kitöltését a védőnők végezték a gondozásba vételkor a várandósok válaszai alapján. A legfőbb biometriai, szociodemográfiai és családtervezési adatokon (9 kérdés) túl a dohányzási (cigarettázási) szokásokat, a környezeti dohányfüstártalmat és a dohányzás ártalmasságának ismeretét vizsgáltuk (9 kérdés). A statisztikai vizsgálatot frekvencia- és kereszttáblás elemzéssel végeztük. Szignifikánsnak fogadtuk el az eredményeket p<0,05 értékkel. Eredmények: A gondozásba vett összes várandós (n = 1761) közül 1548 (87,9%) válaszolt a kérdéseinkre. Mintánk átlagéletkora 30,2 év (min./max.: 15/48) volt, gondozásba vételük átlagosan a várandósság 10,8. hetében (min./max.: 6/34) történt. Jelentős volt a magasabb fokú iskolai végzettséggel (középfokú 46,0%, felsőfokú 42,8%) rendelkezők aránya. A roma nemzetiség aránya 4,2% volt. Tervezett volt a várandósság 88,5%-ban. Soha nem dohányzott 46,5%. Dohányzói anamnézise volt 53,5%-nak. A teljes mintából 10,1% folytatta a dohányzást, de 42,5% korábbi dohányos azt állította, hogy aktuálisan nem cigarettázik. A férjek/élettársak (n = 1493) részéről 30,3%-ban fordult elő környezeti dohányfüstártalom. Minden várandós (98,9%) tisztában volt a dohányzás magzatkárosító hatásával. Következtetés: Nagyvárosi környezetben a magasabb szintű iskolai végzettség hozzájárult a dohányzás magzatkárosító hatásának ismeretéhez. A várandósság alatt tovább folytatott dohányzásban a romák szignifikánsan hátrányosabb helyzetben voltak a nem romákkal szemben. Másodlagos dohányfüstártalom szempontjából nagyon kedvezőtlen volt a férj/élettárs dohányzásának csaknem egyharmados aránya. Az alapellátás a legegyszerűbben a védőnők révén érheti el a dohányzás által különösen veszélyeztetett populációt. Részükre a bemutatott módszerrel a háziorvos és a védőnő az adatok alapján célzottan végezheti a dohányzással kapcsolatban az alapellátás kompetenciájába tartozó szünetelés/leszokás támogatást. Magyarországon a rendszerváltozás során létrehozott vegyes választási rendszerben a választókerületek szavazóinak száma nagy egyenlőtlenségeket mutatott. A demográfiai és társadalmi folyamatok jóvoltából idővel tovább nőttek az eltérések és ezért szükségessé vált a kerületek átrajzolása. 2010 után a parlamenti választások keretei átalakultak és a képviselők számát 386-ról 199-re változtatták. Az átalakítások magukban foglalták az egyéni választókerületek számának 176-ról 106-ra történő csökkentését és a határok teljes átrajzolását is. Ebből adódóan aktuálissá vált egy összehasonlító elemzés, ami a magyar választási rendszer korábbi és aktuális térbeli folyamatait taglalja. A könyv arra a fő kérdésre kereste a választ, hogy milyen területi egyenlőtlenségek jellemezték a magyar választási rendszert 2002 és 2022 között. Ebből adódóan e könyv célkitűzése kettős volt. Egyrészt azt tűzte ki célul, hogy a nemzetközi szakirodalom alapján minél pontosabban leírja a magyar választási rendszerben is megfigyelhető térbeli egyenlőtlenségek sokrétűségét, részletezve az összefüggéseket. Másrészt az is célja volt a könyvnek, hogy a 2002 és 2022 között lezajlott magyar országgyűlési választások alapján kimutassa a területi egyenlőtlenségek mértékét hazánkban és az erre ható mögöttes demográfiai és társadalmi folyamatokra is rávilágítson. A könyv elsősorban a magyar szakirodalomban eddig kevésbé vizsgált elméleti koncepciók, összefüggések bemutatását és az angolszász megközelítés adaptálását végezte el. Ezt követően a kutatás különböző társadalmi, demográfiai és választási adatokat felhasználva matematikai, statisztikai és térinformatikai elemzéseken alapult. Az elemzésből kirajzolódott, hogy 2002 és 2022 között a gerrymandering, a malapportionment és a reactive malapportionment hogyan alakította az egyenlőtlenségeket. Bebizonyosodott, hogy a kerületek átalakítása mellett a politikai reprezentációt a választópolgárok is dinamikusan formálták és a térbeli mintázat idővel átalakult. The individual’s social situation greatly determines the chance of turning their life situation around through moving away. Aims: We investigated the factors that affected the representatives of the three Roma groups researched here (Romungro, Vlach, Boyash) in their moving in the past and in their intentions to move in the future. Methods: A SEM model was developed (N = 570) to analyze the differences between previous movers and non-movers in well-being, socioeconomic status, and social network. We also investigated the effect of the above variables on the intention to move. Data were collected via the “snowball method”. Results: Out of the Vlachs, those who had moved in the past have significantly fewer confidant relatives (p = .021) and also know significantly fewer people pursuing high-prestige vocations (p = .003), moreover, the fewer people pursuing moderate-prestige vocations they know, the more they would like to move away from their present residence (p = .031). Regarding the Boyash, the more favorable their socio-economic situation, the more they would like to move away (p = .007); while regarding the Romungro, the low level of their mental wellbeing (p = .019) and the relatively high number of their confidant relatives constitutes (p = .017) the incentive to change their residence. Conclusions: The spatially mobile Roma who had moved before possess fewer confidant relatives and weak ties. A diskurzus alapja ezekben az esetekben egy olyan szubkultúra, ami rizikótényező, mert egyéni, képességbeli deficithez vezethet. A roma nemzetiséghez tartozás pozitív hatása az identitásra mellőzött, ezért nem képezheti tárgyát a gyógypedagógiai nevelésnek. Következtetések: A vizsgált tanulmányok megközelítése ezért deficitszemléletű, amelyből arra következtetek, hogy a gyógypedagógiai nevelés nem épít a tanulók esetleges kisebbségi identitására, mint erőforrásra. Here we attempt to examine diet quality and adherence to food based dietary guidelines among Hungarian Roma (HR) ethnic minority living in segregated settlements while comparing a sample of Hungarian adults from the general population (HG). Data were obtained from a complex comparative health survey conducted in Northeast Hungary in 2018, including sociodemographic and physical examination data. Dietary data were collected using two non-consecutive 24-h dietary recalls. We assessed diet quality based on using a 13-component Healthy Eating Index-2015 (HEI-2015, range 0–100). Differences in median intakes of food and nutrients and HEI-2015 scores were evaluated by Mann–Whitney test or Kruskal–Wallis test. Quantile regression was used to adjust HEI-2015 scores for socioeconomic factors including age, sex, educational status, and perceived financial status. This analysis included 393 and 415 subjects, aged between 18 to 70 years, of HR and HG populations, respectively. Results showed overall low median HEI-2015 scores for both HR and HG, with significantly lower total score among HR participants (41.6, interquartile range (IQR): 39.5–42.8) compared to HG (47.2, IQR: 45.7–51.1). Scores for individual components, such as intake of fruits, greens and beans, whole grains, seafood, and plant proteins were particularly suboptimal among both groups, but significantly lower among the HR population. Scores for refined grains, sodium, saturated fats and added sugar reflected high intakes of these components but did not differ between study groups. Our findings revealed an unfavorable diet quality among the HR compared to HG and a potentially increased risk for diet-related NCDs. Hungary offers an example to investigate whether social inequalities compromise what a successful vaccine program can achieve. COVID-19 morbidity, mortality, and vaccination coverage were characterized by calculation of indirectly standardized ratios in the Hungarian population during the third pandemic wave at the level of municipalities, classified into deprivation quintiles. Then, their association with socioeconomic deprivation was assessed using ecological regression. Compared to the national average, people living in the most deprived municipalities had a 15-24% lower relative incidence of confirmed COVID-19 cases, but a 17-37% higher relative mortality and a 38% lower vaccination coverage. At an ecological level, COVID-19 mortality showed a strong positive association with deprivation and an inverse association with vaccination coverage (RRVaccination = 0.86 (0.75-0.98)), but the latter became non-significant after adjustment for deprivation (RRVaccination = 0.95 (0.84-1.09), RRDeprivation = 1.10 (1.07-1.14)). Even what is widely viewed as one of the more successful vaccine roll outs was unable to close the gap in COVID-19 mortality during the third pandemic wave in Hungary. In the last two decades of educational policy in Hungary, elements aimed at inclusion and maintaining selectivity have been mixed together. In our cross-sectional study, we present the situation of young Roma people in the educational system of Hungary in 2016. In addition to outlining the overall picture (employment and education), we also look at softer indicators, such as the educational plans and motivations of Roma youth. We use the Hungarian Youth Research 2016 database (respondents aged between 15 and 29), including Roma or dual identity youth (N=328). The probability sample of 8000 persons is representative for age, district and type of settlement (N=8000). Our results have shown that inequalities are climbing upwards. While inclusion and catching-up with non-Roma students can be detected in primary and secondary vocational education (i.e., without the baccalaureate), there is a significant gap in terms of obtaining a baccalaureate and entering tertiary education. Jelen tanulmány célja a járás népesedési folyamatainak megértetése, amelyre a jövőben a járás településeinek társadalmi felzárkózás szempontú összehasonlító elemzése épül majd. A járás lakossága 2011 óta apad, az országos átlagokhoz képest a jövedelemszint és az adófizetők aránya alacsony, rosszabbak az egészségügyi mutatók és magasabb a korai halálozás aránya. Háromból két gyermek hátrányos helyzetű, miközben a fiatalok túlnyomó többsége végzettség megszerzése nélkül lemorzsolódik a középfokú oktatásból. A romák magas aránya önmagában nem magyarázhatja a romló gazdasági-társadalmi adatokat, meggyőződésünk, hogy a gazdasági teljesítménynek nincs etnikai specifikuma, oka inkább a társadalmi mobilitási lehetőségeknek az elmúlt két évtizedben bekövetkező jelentős beszűkülésében keresendő. Az ország számos más, magas roma lakosság-arányú járásához hasonlóan itt is jellemző a nem-roma népesség még nagyobb arányú elvándorlása, és a romák korábbi gyermekvállalási hajlandósága, magasabb fertilitása, melynek következtében e térségek elszlömösödnek, amivel a társadalmi mobilitási esélyek tovább szűkülnek. A felzárkóztató programoknak, gazdaságélénkítő beruházásoknak és a helyi roma képviselet aktív szerepvállalásának köszönhetően a járásban megindultak reménykeltő változások, például az iskolai végzettség és foglalkoztatottság növekedése, vagy a veszélyeztetett gyermekek számának csökkenése tekintetében. Kérdés, hogy az eddigi eredmények mennyire fenntarthatók, illetve vajon lehet-e számítani arra, hogy a térség idővel önfenntartó társadalmi felzárkózási pályára állhat-e. Two out of three children are disadvantaged, while the vast majority of young people drops out of secondary school without any qualification. The higher-than-average proportion of Roma population alone does not explain worsening economic output, we are convinced that economic performance has nothing to do with ethnic origin. Reasons are rather to be found in the deterioration of social mobility of the past two decades. Similarly to areas with a higher proportion of Roma population, the District also undergoes an exodus of non-Roma besides a higher fertility rate and earlier desire for child-bearing of the Roma, resulting in ghettoization, thereby further diminishing chances of social mobility. Thanks to social inclusion and recovery programs, as well as the commitment of local Roma stakeholders, promising changes are coming true with an increase in qualification and employment levels and a decrease in the number of disadvantaged children, we wonder whether development is sustainable in the long run, and also, whether the District has a potential to independently self-sustain social development. The central question is how social ascension through educational mobility and the concomitant movement between different social worlds influence the habitus. Under what conditions does the habitus become destabilised as a result of upward mobility? The paper benefits from the empirical results of a 3-year study during which our research team has conducted 153 life history interviews with first-generation graduates in Hungary. The inclusion criteria for the sample of our study was that respondents had to complete college or university despite none of their parents have had a university degree. We identified the most important factors that contribute to the destabilisation of the habitus, either temporary or permanent. We examined the social and geographical range of our respondents’ education-driven mobility; the speed and the destination of their mobility (field of occupation); their belonging to the majority or a minority group; and the mobility aspirations of their family of origin (or the lack thereof). We explore the effect of these factors through an intersectional lens. We demonstrate that the unique combination and intersection of these factors greatly affect the subjective experience of mobility. However, some biographical conditions and contingencies also play a role in the outcome of upward mobility. According to our results, the dislocation of habitus is a particularly common experience for our Roma interviewees, at least at some stage of their mobility trajectory. Methods The spatial distribution of morbidity, mortality and case fatality was mapped using hierarchical Bayesian smoothed indirectly standardised ratios. Indirectly standardised ratios were used to evaluate the association between deprivation and the outcome measures. We looked separately at morbidity and mortality in the 10 districts with the highest and 10 districts with the lowest share of Roma population. Results Compared with the national average, the relative incidence of cases was 30%-36% lower in the most deprived quintile but the relative mortality and case fatality were 27%-32% higher. Expressed as incidence ratios relative to the national average, the most deprived municipalities had a relative incidence ratio of 0.64 (CI: 0.62 to 0.65) and 0.70 (CI: 0.69 to 0.72) for males and females, respectively. The corresponding figures for mortality were 1.32 (CI: 1.20 to 1.44) for males and 1.27 (CI: 1.16 to 1.39) for females and for case fatality 1.27 (CI: 1.16 to 1.39) and 1.32 (CI: 1.20 to 1.44) for males and females, respectively. The excess death rate (per 100 000) increased with deprivation levels (least deprived: 114.12 (CI: 108.60 to 119.84) and most deprived: 158.07 (CI: 149.30 to 167.23)). The 10 districts where Roma formed the greatest share of the population had an excess mortality rate 17.46% higher than the average for the most deprived quintile. Conclusions Those living in more deprived municipalities had a lower risk of being identified as a confirmed COVID-19 case but had a higher risk of death. Since 2010, a growing number of reports in grey literature and anecdotal information among professionals have become available on NPS use among previously unaffected groups, such as people living in rural, socioeconomically deprived communities. In our research, we aimed to explore NPS use among these communities. Methods: We conducted a mixed method research with convergent parallel design. Data collection took place in 2017 in marginalised communities in villages in two regions in Hungary, where 150 questionnaires were recorded and 50 interviews were conducted with current NPS users. Results: According to the survey results, NPS is very easy to access, synthetic cannabinoid receptor agonist (SCRA) are easily bought in marginalised rural communities (79% found SCRA easy to obtain). Both SCRA and synthetic cathinones are used regularly; 57% of SCRA users and 37% of synthetic cathinone users used the respective substance at least once a week in the past 30 days. Besides NPS, sedative use (without prescription) and alcohol consumption are common among the respondents. 17% of the sample has already injected NPS. The overwhelming majority of the respondents rated regular consumption of NPS as “very dangerous” (SCRA: 75%, synthetic cathinones 72%). NPS users have limited knowledge of consequences and the social and health treatment options available. Most themes in the interviews are associated with surviving stress, crisis and anxiety, as well as the wish to escape from insecurity and chaotic life. Positive effects of substance use (community, joy, energy) are rarely present. Conclusion: People who use drugs (PWUD) living in these rural communities face the consequences of the rural risk environment: easy access to NPS, inadequate access to services, poor labour market situation and attributions of marginalised groups, for example disaffiliation. NPS use is not a recreational activity in this population; individuals mainly use NPS to get away from reality, problems, pain, poverty and marginalisation. NPS use is a survival strategy. This study was designed to estimate the genetic susceptibility of the Hungarian Roma (HR) population to essential hypertension (EH) and compare it to that of the general (HG) population. Twenty EH associated SNPs (in AGT, FMO3, MTHFR-NPPB, NPPA, NPPA-AS1, AGTR1, ADD1, NPR3-C5orf23, NOS3, CACNB2, PLCE1, ATP2B1, GNB3, CYP1A1-ULK3, UMOD and GNAS-EDN3) were genotyped using DNA samples obtained from HR (N = 1176) and HG population (N = 1178) subjects assembled by cross-sectional studies. Allele frequencies and genetic risk scores (unweighted and weighted genetic risk scores (GRS and wGRS, respectively) were calculated for the study groups and compared to examine the joint effects of the SNPs. The susceptibility alleles were more frequent in the HG population, and both GRS and wGRS were found to be higher in the HG population than in the HR population (GRS: 18.98 ± 3.05 vs. 18.25 ± 2.97, p<0.001; wGRS: 1.4 [IQR: 0.93-1.89] vs. 1.52 [IQR: 0.99-2.00], p<0.01). Twenty-seven percent of subjects in the HR population were in the bottom fifth (GRS ≤ 16) of the risk allele count compared with 21% of those in the HG population.
