However, existing methods based on case numbers, hospital admissions or fatalities suffer from multiple measurement biases and temporal lags due to high test positivity rates or delays in symptom development or administrative reporting. Alternative methods such as web search and social media tracking are less directly indicating epidemic prevalence over time. We instead record age-stratified anonymous contact matrices at a daily resolution using a longitudinal online-offline survey in Hungary during the first two waves of the COVID-19 pandemic. This approach is innovative, cheap, and provides information in near real-time for estimating R_t R t at a daily resolution. Moreover, it allows to complement traditional surveillance systems by signaling periods when official monitoring infrastructures are unreliable due to observational biases. first year of the pandemic. The study population consisted of outpatient adults with mild or no COVID-19 symptoms and was divided into subpopulations with different levels of exposure. Among the subpopulation without known previous COVID-19 contacts, 4143 patients were investigated. Of the subpopulation with known COVID-19 contacts, 594 patients were investigated. IgG- and IgA-seroprevalence and RT-PCR positivity were determined in context with COVID-19 symptoms. Our results suggested no significant age-related differences between participants for IgG positivity but indicated that COVID-19 symptoms occurred most frequently in people aged between 20 and 29 years. Depending on the study population, 23.4–74.0% PCR-positive people (who were symptomless SARS-CoV-2 carriers at the time of the investigation) were identified. It was also observed that 72.7% of the patients remained seronegative for 30 days or more after their first PCR-positive results. This study hoped to contribute to the scientific understanding of the significance of asymptomatic and mild infections in the long persistence of the pandemic. The severity of these surges varied due to the different virulences of the variants. In a single-center, retrospective, observational study, we aimed to assess and compare morbidities and mortality rates across the epidemic waves I to IV with special regard to hospitalized, critically ill patients. A significant difference was found between the surges with regard to morbidity (p < 0.001) and ICU mortality (p = 0.002), while in-hospital mortality rates (p = 0.503) did not differ significantly. Patients under invasive ventilation had a higher incidence of bloodstream infection (aOR: 8.91 [4.43–17.95] p < 0.001), which significantly increased mortality (OR: 3.32 [2.01–5.48]; p < 0.001). Our results suggest that Waves III and IV, caused by the alpha (B.1.1.7) and delta (B.1.617.2) variants, respectively, were more severe in terms of morbidity. The incidence of bloodstream infection was high in critically ill patients. Our results suggest that clinicians should be aware of the risk of bloodstream infection in critically ill ICU patients, especially when invasive ventilation is used. Here we report on analysing genome sequences of SARS-CoV-2 from the first two waves of the epidemic in 2020 in Hungary, mirroring a suppression and a mitigation strategy, respectively. Our analysis reveals that the two waves markedly differed in viral diversity and transmission patterns. Specifically, unlike in several European areas or in the USA, we have found no evidence for early introduction and cryptic transmission of the virus in the first wave of the pandemic in Hungary. Despite the introduction of multiple viral lineages, extensive community spread was prevented by a timely national lockdown in March 2020. In sharp contrast, the majority of the cases in the much larger second wave can be linked to a single transmission lineage of the pan-European B.1.160 variant. This lineage was introduced unexpectedly early, followed by a two-month-long cryptic transmission before a soar of detected cases in September 2020. Epidemic analysis has revealed that the dominance of this lineage in the second wave was not associated with an intrinsic transmission advantage. This finding is further supported by the rapid replacement of B.1.160 by the alpha variant (B.1.1.7) that launched the third wave of the epidemic in February 2021. Overall, these results illustrate how the founder effect in combination with cryptic transmission, instead of repeated international introductions or higher transmissibility, can govern viral diversity. A járvány és a járvány- vagy vírusválság a mindennapi hírek állandósult velejárói 2020 eleje óta. A járvánnyal kapcsolatos jelenségeknek és hatásoknak a megismerése, az összefüggések értelmezése, a megoldási lehetőségek kidolgozása és a reális veszélyek elhárítása számos tudományterület célkeresztjébe került. Megszámlálhatatlan a tudományos közlemények, összefoglaló művek, ismeretterjesztő írások száma, amelyek valóban a járvány és a válság összetettségének, egyúttal egyedi vonásainak magyarázatára épülnek. Közöttük markáns csoportot alkotnak azok a publikációk, amelyek a járvány térbeli, területi és/vagy földrajzi jellemzőit tanulmányozzák. Ezek felhasználásával a jelen könyv az egyes fejezetek segítségével áttekintést nyújt a járvány legfontosabb egészséghatásairól és egészségügyi következményeiről, elsősorban az egészségföld-rajz elméleti és módszertani eszköztárát kiaknázva. Az egyes jelenségek és folyamatok leírásában a nemzetközi gyakorlat bemutatása mellett az elsődleges cél a hazai példák elemzése és értékelése. Az egészségföldrajzi szemelvények tehát egy–egy jellegzetes járványjelenség kiragadásával mutatják be a COVID-19 mint társadalmi és egészségügyi jelenség térbeliségét, de egyúttal kijelölik a vizsgálatok korlátait, határait is. A könyvfejezetek nem tudnak kitérni minden lehetséges és várható járványjelenség és járványfolyamat bemutatására, de természetesen igyekeznek a leginkább releváns kutatási eredményekre, ismeretekre, egyáltalán a felhalmozott tudásbázisra utalni.
