Ezek érintik a vérnyomás emelkedésében és a következményes szervkárosodásokban (úgynevezett hypertonia mediálta szervkárosodás [hypertension-mediated organ damage] - HMOD) szerepet játszó kórélettani folyamatokat, a hypertonia epidemiológiáját, a társuló cardiovascularis kockázati tényezőknek és társbetegségeknek a hypertoniás betegekre gyakorolt hatását, a vérnyomáscsökkentő nem gyógyszeres és gyógyszeres kezelés hatékonyságát, valamint a hypertonia genetikáját. Ezek tették lehetővé a célvérnyomás pontosabb meghatározását is. Feladatunk tehát az volt, hogy ezeket az eredményeket beépítsük a korábbi irányelv szövegébe úgy, hogy egyúttal gyakorlatiasabb szempontokat is érvényesítsünk, és a hypertonia diagnosztikájára, kezelésére vonatkozó ajánlást közvetlenebb módon fogalmazzuk meg. A Magyar Hypertonia Társaság (MHT) 2018. évi irányelveinek szerkezete nem tér el lényegesen a korábbitól. Az Egyesült Államokban több szív- és érrendszeri halálesetért tehető fe - lelőssé, mint bármely más befolyásolható rizikófaktor, és a dohányzás után a második helyen áll a bármilyen okból bekövetkező halálozás megelőzhető okainak listáján. A helyzet hazánkban sem különbözik. Magyarországon hozzávetőlegesen 3,5 millió hypertoniás személy él, és ez az igen magas prevalencia jelentősen hozzájárul a hazai rossz szív- és érrendszeri mortalitási és morbiditási mutatókhoz. A hypertonia felismerése, a gyógyszeres kezelés beállítása és a betegek gondozása elsősorban alapellátási feladatkör. Amellett, hogy ez nagy felelősséget ruház a családorvosokra, ugyanakkor hálás feladat is, miután életmódváltással és a rendelkezésre álló gyógyszerekkel a hypertonia az esetek jelentős részében kiválóan kezelhető, így a szövődmények, elsősorban a stroke prevalenciája minimalizálható a praxisokban.
