A második COVID–19-hullám tetőfokán idegsebészeti posztoperatív őrzőnkben 11, intenzív ellátást igénylő beteget kezeltünk. A 11 betegből 7 COVID–19-pozitív lett, és közülük 4 betegnél még C. difficile fertőzés is kialakult. A 4-ből 2 beteget elveszítettünk. Bár megfigyeléseinket kevés beteg ellátása során gyűjtöttük, úgy ítéljük meg, hogy súlyos állapotú betegeknél a SARS-CoV-2 és a C. difficile együttes fertőzése többletkockázatot jelent. A COVID–19-ben alkalmazott, széles spektrumú szisztémás antibiotikumok használata növelheti a C. difficile infekció rizikóját. Jól átgondolt infekciókontroll, a megfelelő higiéné betartása, az alkoholos kézfertőtlenítők mellett a rendszeres szappanos kézmosások mérsékelhetik a nosocomialis C. difficile fertőzések számát. Conical intersections (CIs) of different electronic potential energy surfaces lead to the most singular non-adiabaticity and have been intensely investigated. The coupling of light and matter induces conical intersections, which are termed light-induced conical intersections (LICIs). There are two kinds of LICIs, those induced by classical (laser) light and those by quantum light like that provided by a cavity. The present work reviews the subject of LICIs, discussing the achievements made so far. Particular attention is paid to comparing classical and quantum LICIs, their similarities and differences and their relationship to naturally occurring CIs. In contrast to natural CIs, the properties of which are dictated by nature, the properties of their light-induced counterparts are controllable by choosing the frequency and intensity (or coupling to the cavity) of the external light source. This opens the door to inducing and manipulating various kinds of non-adiabatic effects. Several examples of diatomic and polyatomic molecules are presented covering both dynamics and spectroscopy. The computational methods employed are discussed as well. To our opinion, the young field of LICIs and their impact shows much future potential. Data gathered by multipurpose track recording vehicles matched the EN 13,848 requirements. Track Quality Index (TQI) measurement systems (The Federal Railroad Administration (FRA), the Netherlands’, and the Chinese TQI) are considered where three types of predetermined segment techniques: separate, moving, and Zero-crossings segmentation are employed. The importance of track segmentation in quality assessment, which affects maintenance planning, is shown by key findings. For heterogeneous data, the TQIs might be deceptive, highlighting the need for alternatives. The robustness of the Zero-crossings method makes it possible to analyze deterioration factors in great detail and in some efficient way. Longer analytical segments and smoothing of the data improved precision. Based on empirical data, we advise considering a Zero-crossings strategy for precise and efficient track-quality evaluations. With the help of this study, track quality can be better evaluated for train systems. AB - It has been known that electronic conical intersections in a molecular system can also be created by laser light even in diatomics. The direct consequence of these light-induced degeneracies is the appearance of a strong mixing between the electronic and vibrational motions, which has a strong fingerprint on the ultrafast nuclear dynamics. In the present work, pump and probe numerical simulations are performed with the NaH molecule involving the first three singlet electronic states (X1 sigma+(X), A1 sigma+(A) and B1 pi(B)) and several light-induced degeneracies in the numerical description. To demonstrate the impact of the multiple light-induced non-adiabatic effects together with the molecular rotation on the dynamical properties of the molecule, the dissociation probabilities, kinetic energy release spectra (KER) and the angular distributions of the photofragments were calculated by discussing the role of the permanent dipole moment as well.It has been known that electronic conical intersections in a molecular system can also be created by laser light even in diatomics. Pituitary adenylate cyclase-activating polypeptide (PACAP) can aid healing by decreasing inflammation, apoptosis and oxidative stress. In addition to hematological and hemorheological tests, we examined the biomechanical and histological features of vascular anastomoses with or without PACAP addition and/or using a hemostatic sponge (HS). End-to-end anastomoses were established on the right femoral arteries of rats. On the 21st postoperative day, femoral arteries were surgically removed for evaluation of tensile strength and for histological and molecular biological examination. Effects of PACAP were also investigated in tissue culture in vitro to avoid the effects of PACAP degrading enzymes. Surgical trauma and PACAP absorption altered laboratory parameters; most notably, the erythrocyte deformability decreased. Arterial wall thickness showed a reduction in the presence of HS, which was compensated by PACAP in both the tunica media and adventitia in vivo. The administration of PACAP elevated these parameters in vitro. In conclusion, the application of the neuropeptide augmented elastin expression while HS reduced it, but no significant alterations were detected in collagen type I expression. Elasticity and tensile strength increased in the PACAP group, while it decreased in the HS decreased. Their combined use was beneficial for vascular regeneration. We have followed the standards, recommendations, and protocols during the designprocess. The result is an application framework which fulfills the duty of an XXI. century smart solutionsapplication that could monitor the health of people and even help to live a healthier life avoiding chronicdisorders e.g., obesity and/or diabetes. We focused on predicting and classifying on COVID-19 disease tooaccording to the collected information and research results. We achieved tihs project using our own deviceslike a Samsung Galaxy Watch 2 whch comes integrated Tizen OS and gave the opportunity to develop adata sending application on it. On the other hand we worked with smartbands like the Xiaomi Mi Band 6but it does not have programable OS so we needed to find a solution to collect data from it but his case isdiscussed in the paper later. We would like to implement this project live with the Clinic of our universityto get more real cases and data.
