A közlemény négy, rutinszerűen mért analitra fókuszál: szérum-kreatininszint, májtranszamináz-aktivitások, albuminszint. A transzamináz-aktivitások (GOT/AST, GPT/ALT) meghatározásakor kritikus a piridoxál-foszfát (B6-vitamin) jelenléte a reagensben. Ennek hiányában a mért értékek 15–30%-kal alacsonyabbak lehetnek. Ez téves diagnózishoz vezethet, különösen a májfibrosis becslésére szolgáló FIB4-pontszám számításakor. Ezért piridoxál-foszfátot tartalmazó reagensek alkalmazása javasolt a transzamináz-aktivitások mérésére. Orv Hetil. 2026; 167(6): 211–216. | Despite advances in modern medicine, classic laboratory parameters are still crucial pillars of clinical decision-making. The report focuses on four routinely measured analytes: serum creatinine levels, liver transaminase activities and al- bumin levels. In 2024, these accounted for more than 15% of laboratory determinations in Hungary. Laboratories measure serum creatinine levels using two methods: the Jaffé or the enzymatic method. Although both are suitable for estimating the glomerular filtration rate (GFR), the results may differ by 5–14% from each other. According to the professional opinion, both methods can be used, but they should be indicated on the report. The presence of pyridoxal phosphate (vitamin B6) in the reagent is critical for the determination of transaminase activities (GOT/ AST, GPT/ALT). In its absence, the measured values may be 15–30% lower, which may lead to misdiagnosis, espe- cially when calculating the FIB4 score for estimating the severity of liver fibrosis. Therefore, the use of reagents containing pyridoxal phosphate is recommended for measuring transaminase activities. In the case of albumin deter- mination, the use of bromocresol red reagent is recommended instead of bromocresol green, as the former gives inaccurate results in liver disease and renal failure. Method changes should be made in consultation with clinicians, and it should be noted that the LOINC codes (an international standard for parameter identification) for the tests may change.}, year = {2026}, eissn = {1788-6120}, pages = {211-216}, orcid-numbers = {Vásárhelyi, Barna/0000-0003-0055-7346; Bereczky, Zsuzsanna/0000-0002-1483-3703} } @article{MTMT:36934363, title = {Venous Blood Cell Ratios as Predictors of Reperfusion Outcomes in Ischemic Stroke: A Systematic Review and Meta-analysis}, url = {https://m2.mtmt.hu/api/publication/36934363}, author = {Szegedi, István and Éles, Zsolt Barnabás and Nagy, Attila Csaba and Bagoly, Zsuzsa}, doi = {10.1007/s40120-025-00881-0}, journal-iso = {NEUROL THERAP}, journal = {NEUROLOGY AND THERAPY}, volume = {-}, unique-id = {36934363}, issn = {2193-8253}, year = {2026}, eissn = {2193-6536}, pages = {Online ahead of print.}, orcid-numbers = {Szegedi, István/0000-0003-4181-4361; Nagy, Attila Csaba/0000-0002-0554-7350; Bagoly, Zsuzsa/0000-0001-5314-5607} } @article{MTMT:37057169, title = {Autoantibodies in the diagnostics, prognostics and follow-up of primary biliary cholangitis}, url = {https://m2.mtmt.hu/api/publication/37057169}, author = {Antal-Szalmás, Péter and Bencze, Dóra and Demeter, Sarolta and Pénzes-Daku, Krisztina and Szabó, Lilla and Tóth, Beáta and Földesi, Róza and Papp, Mária and Nagy, Gábor}, doi = {10.1016/j.jtauto.2026.100366}, journal-iso = {J TRANSL AUTOIMMUN}, journal = {JOURNAL OF TRANSLATIONAL AUTOIMMUNITY}, volume = {12}, unique-id = {37057169}, year = {2026}, eissn = {2589-9090}, orcid-numbers = {Pénzes-Daku, Krisztina/0000-0003-0334-7571; Papp, Mária/0000-0003-3662-4010; Nagy, Gábor/0000-0002-2943-2750} } @article{MTMT:37325625, title = {The Methanesulfonamide Group: Bright and Dark Sides of hERG Potassium Channel Inhibition}, url = {https://m2.mtmt.hu/api/publication/37325625}, author = {Kovács, Zsigmond Máté and Óvári, József and Magyar, János and Bányász, Tamás and Nánási, Péter Pál and Horváth, Balázs and Balogh, Gábor and Szentandrássy, Norbert}, doi = {10.3390/ph19060882}, journal-iso = {PHARMACEUTICALS-BASE}, journal = {PHARMACEUTICALS}, volume = {19}, unique-id = {37325625}, abstract = {Our review focuses on methanesulfonamide-containing compounds, a well-characterized class of high-affinity blockers of the hERG potassium channel, which plays a critical role in cardiac repolarization by mediating the cardiac IKr. These compounds, which include notable class III antiarrhythmic drugs such as dofetilide and d-sotalol, block the hERG channel in its open state by binding within the inner vestibule. This interaction is particularly strong with some residues and the compounds form hydrogen bonds with others. This binding results in high-affinity inhibition with slow dissociation kinetics, frequently leading to drug trapping and prolonged action potential duration (APD). This can predispose patients to arrhythmias, including Torsades de Pointes. Beyond cardiac drugs, there are several non-cardiac methanesulfonamide drugs that also block the hERG channel. This causes pro-arrhythmic side effects despite their primary indications. The clinical significance of these effects, especially in patients with impaired drug elimination, is that accumulation increases the risk of arrhythmia. The objective of forthcoming research endeavors is to mitigate hERG affinity, with the aim of reducing pro-arrhythmic risks while maintaining therapeutic efficacy. This means structural modifications that seek to remove or modify the methanesulfonamide group. Machine learning also emerged as promising tool for exploring drug–protein interactions. It is evident that the methanesulfonamide moiety plays a pivotal role in the structural basis of hERG blockade. However, it should be noted that this moiety does not necessarily represent a universal pharmacophore. This interventional clinical study aimed to evaluate the oral health status of hyperlipidemic patients receiving lipid-lowering therapy for at least 6 months and the effects of non-surgical and surgical dental treatments on serum C-reactive protein (CRP) levels and lipid markers. Methods: Twenty-eight patients with controlled hyperlipidemia and 18 healthy controls were enrolled in the study. All participants underwent dental examinations (clinical evaluation, X-ray imaging, and microbial analysis of subgingival and supragingival plaque samples) at baseline. Hyperlipidemic patients received periodontal, endodontic, and dentoalveolar surgical treatments. Serum CRP and lipid parameters were assessed at baseline, 1 week, and 3 months, while subgingival and supragingival plaque samples were analyzed at baseline and 3 months after completing dental treatments. Results: At the 3-month follow-up, clinical periodontal characteristics, including the plaque index, gingival index, and periodontal probing depth, improved significantly (p < 0.05). A significant shift in microflora was observed in both subgingival and supragingival plaque samples (p < 0.05), alongside improvements in periodontal values and a significant reduction in serum CRP levels (p < 0.05). Serum cholesterol levels decreased significantly, while moderate improvements in serum triglycerides, low-density lipoprotein, and high-density lipoprotein levels were observed but were not statistically significant (p > 0.05). However, prompt evaluation of heparin inhibitory effects (i.e., anti-Xa activity) is important in cases of life-threatening bleeding, need for urgent surgery or acute thromboembolism under LMWH treatment. We aimed to establish a simple and reliable point-of-care method for the detection of enoxaparin. Methods: Eighty patients under enoxaparin therapy and ten healthy volunteers without any anticoagulant treatment were enrolled. Simultaneous measurements of anti-Xa activity using the chromogenic method and clotting times in the absence and presence of polybrene using viscoelastometric assays containing Russell's viper venom (RVV-test) were performed on the ClotPro device. Results: Among the measured and derived RVV-test parameters, the ratio of the RVV clotting times (RVV CT) detected in the absence and presence of polybrene showed the best statistically significant correlation with anti-Xa activity (r = 0.774, p < 0.001). Based on ROC analysis, we designated RVV CT ratios of 1.02, 1.23 and 1.6 as the best cut-off values for separating anti-Xa ranges below and above 0.3 and 0.6 IU/mL, respectively. If the RVV CT ratio is below or above 1.23, the anti-Xa activity is suggested to be below 0.6 IU/mL or above 0.3 IU/mL with high certainty, respectively. Further differentiation is possible if the RVV CT ratio is measured below 1.02 or above 1.6. In these cases, the measured anti-Xa values are below 0.3 IU/mL or above 0.6 IU/mL, respectively, with high probability and good predictive values.
