The aim of this study was to establish relationship between concentrations of soluble Human Leukocyte Antigen-G (sHLA-G) in maternal serum and amniotic fluid at 16-22 weeks of gestation and the sonographic measurements of the fetal and placental growth. Materials and methods: sHLA-G in serum and amniotic fluid, as well as fetal biometric data and placental volume and perfusion indices were determined in 41 uncomplicated singleton pregnancies. Results: The sHLA-G levels were unchanged both in amniotic fluid and in serum during mid-pregnancy. The sHLA-G level in serum correlated positively with amniotic sHLA-G level (β=0.63, p<0.01). Significant correlation was found between serum sHLA-G level and abdominal measurement (β=0.41, p<0.05) and estimated fetal weight (β=0.41, p<0.05). The amniotic sHLA-G level and placental perfusion (VI: β = -0.34, p<0.01 and VFI: β = -0.44, p<0.01, respectively) were negatively interrelated. A low amniotic sHLA-G level was significantly associated with nuchal translucency (r = -0.102, p<0.05). Célkitűzés: Kutatásunk során a perimenopausalis tünetek kialakulásának időpontját, jellegét és a mentális jóllétet vizsgáltuk, figyelembe véve a COVID–19-járvány okozta hatásokat (oltások, fertőzöttség). Módszer: Vizsgálatunkban menopausában lévő nőket kérdeztünk meg a változókort kísérő tüneteikről (n = 264, átlagéletkor: 50,69 év, SD = 4,88). A résztvevők szociodemográfiai, valamint nőgyógyászati kérdésekre és a következő pszichológiai kérdőívekre válaszoltak: Kapcsolati Elégedettség Skála, Multidimenzionális Észlelt Társas Támogatás Kérdőív, Énhatékonyság Kérdőív, Élettel Való Elégedettség Skála, Spielberger-féle Állapot- és Vonásszorongás Kérdőív, Beck Depresszió Kérdőív. Továbbá a koronavírus-járvánnyal kapcsolatos stresszorok feltárását célzó saját összeállítású kérdőív is szerepelt a kérdőívcsomagban, amelyet online felületen tettünk elérhetővé. Eredmények: A perimenopausában lévő nők 12,9%-a tartozott a középsúlyos depresszió kategóriájába, és súlyos depressziót jelző értéket mutatott a résztvevők 8%-a. Az orvosi segítségért forduló, változókorban lévő nők alacsonyabb szintű szorongással és depresszióval küzdöttek. A COVID–19-hatásokat, valamint a fertőzöttség és az oltások felvételének stresszhatásait vizsgálva azt találtuk, hogy a koronavírus-járvány idején a szociális távolságtartás, az izoláció, a fertőzéstől való félelem volt a legmarkánsabb stresszor. A fertőzésen már átesettek csoportjában szignifikánsan kisebb depressziót (t(154,876) = 2,333, p = 0,021) és szorongást (t(186) = 2,050, p = 0,042) jelző értéket és az élettel való nagyobb elégedettségre (t(186) = –2,283, p = 0,024) utaló értéket kaptunk, mint azoknál, akik még nem fertőződtek meg. Az oltások felvétele nem csökkentette a szorongás- és depresszióértékeket. Megbeszélés és következtetés: Az életvitelt megnehezítő változókor gyakoribb szorongással és depresszióval, valamint kisebb énhatékonysággal és élettel való elégedettséggel járt együtt. A pandémia megjelenése tartósan, többnyire negatívan befolyásolta a vizsgálatban részt vevők mentális egészségét. A változókorban lévő nők gondozása során elengedhetetlen a pszichés tünetek felmérése a páciensek életminőségének javítása érdekében. Methods: In the present retrospective study, women with PCOS having normal fasting insulin sensitivity (n = 88) were compared with women with PCOS showing impaired insulin sensitivity (n = 46) using the HPCOS (Hungarian Polycystic ovarian syndrome) database. Results: The impaired insulin sensitivity group has significantly higher body mass index (BMI) and HOMA index than the normal fasting insulin sensitivity group (BMI (kg/m2): 22.0 vs. 28.1, p < 0.0001, HOMA index: 0.96 vs. 2.38, p < 0.0001). The sex hormone binding globulin (SHBG) level was significantly lower, and the free androgen index proved itself significantly higher in the impaired insulin sensitivity group (p < 0.05). Linear regression analysis showed a negative association of BMI with SHBG levels in both groups, while BMI had a positive correlation with insulin concentrations in both groups. However, the SHBG levels were negatively associated with insulin concentrations in the impaired insulin sensitivity group, but this inverse association could not be observed in the normal fasting insulin sensitivity group. Human leukocyte antigen-G (HLA-G) is a member of the class Ib histocompatibility family, which is expressed in the placenta [3]. Soluble (s)HLA-G isoforms include HLA-G1 and HLA-G5-7. The sHLA-G isoforms are detectable in both extravillous and villous cytotrophoblasts, as well as in the chorion membrane and syncytiotrophoblasts [3,4]. The sHLA-G isoforms play critical roles in trophoblast invasion, remodelling of spiral arteries, fetal development, and materno-fetal immune tolerance [3,5]. Increasingly, findings support the role of HLA-G in complications associated with pregnancy. The level of total soluble and membrane-bound HLA-G decreased significantly in cases of preeclamptic placenta and maternal serum compared with placenta and maternal serum of pregnant women with no relevant complications in [6,7]. An increased level of HLA-G5 in the placenta [8] and in maternal serum [9] was observed in preeclampsia (PE), whereas a diminished sHLA-G1 level was described in PE [9] in the third trimester. Placental protein-13 (PP13) is a member of the galectin family and is synthesized in syncytiotrophoblasts [11]. Moreover, it contributes to immunoregulation by inducing apoptosis of maternal T cells [12,13]. PP13 can predict preeclampsia in the first trimester [14,15].
