AB  - The destabilization of transthyretin (TTR) tetramers underlies the pathogenesis of ATTR amyloidosis, a disease manifesting in form of toxic amyloid deposits. Here we investigate two clinically relevant TTR variants, H88R and I107V, found among ATTR patients in the Carpathian Basin. While the I107V mutation exerts only a mild effect on tetramer assembly and monomer stability, it decreases the denaturation midpoint and enhances aggregation propensity at physiological pH, against both wild-type and monomeric TTR (MTTR) backgrounds. Structural analysis revealed that the absence of the methyl group of residue 107 perturbs the hydrophobic trapping of F87 across the primary tetramerization interface. We also found that the far more disruptive H88R mutation that fully abolishes tetramer formation and yields a highly amyloidogenic monomer also acts through the disarray of the F87-centered inter-chain contact. Its equilibrium ensemble includes unfolded components and displays reduced denaturation entropy, traits suggestive of a primed aggregation-prone state. Simulations reveal that the H88R mutation leads to the partial disordering of the EF helix/loop, which in wild-type TTR participates in a Trp-cage-like architecture (centered on W79) shielding the hydrophobic core of the monomeric form. Our results suggest that the H88R variant may serve as a more physiologically relevant model of aggregation-prone TTR than the widely used MTTR double mutant, which does not show amyloidogenic propensity at physiological pH. Based on their physicochemical properties and position with respect to the determined interaction loci, we offer explanation for the phenotypic presentation of D18G, A25T, and Y114H and H88R monomerizing mutations.

AB  - Az autológ haematopoeticus őssejt-transzplantáció magyarországi alapjai az 1980-as évek végén kezdtek kibontakozni, mára már 6 hazai központban is a hematológiai terápia rutinszerűen alkalmazott része. Az eljárás jelentős szerepet játszik a myeloma multiplex, a Hodgkin- és non-Hodgkin-lymphomák kezelésében. Továbbá a nem hematológiai betegségek kezelésében is egyre több indikációja van. Bár az autológ haematopoeticus őssejt-transzplantáció az allogén őssejt-transzplantációnál kevesebb akut szövődménnyel jár, a terápia során számos rövid és hosszú távú cardiovascularis mellékhatás fordulhat elő, főként a nagy dózisú kemoterápiás előkezelés és az alapbetegség kezelése kapcsán a megelőzően alkalmazott, potenciálisan cardiotoxicus onkológiai terápiák következtében. Mind az akut, a peritranszplantációs időszak és az azt követő 100 nap alatt bekövetkező, mind pedig a késői (\>100 nap) cardiovascularis események előfordulása jelentősen gyakoribb, mint a hasonló korú általános populációban. A transzplantáció során fellépő major adverz cardiovascularis események limitálhatják a terápia sikerét, valamint ronthatják a hematológiai alapbetegség prognózisát és a kezelt beteg életminőségét is. Éppen ezért ezek mielőbbi felismerése, előrejelzése fontos, a primer és szekunder prevenció alapját képezi, növelve a betegek életkilátásait és életminőségét. Közleményünk célja az autológ haematopoeticus őssejt-transzplantáció potenciális cardiovascularis mellékhatásainak, ezek monitorozásának és megfelelő ellátásának összefoglalása az aktuális nemzetközi hematológiai és onkokardiológiai irányelvek tükrében. Orv Hetil. 2026; 167(13): 490–498. | The roots of autologous hematopoietic stem cell transplantation in Hungary began to develop in the late 1980s, and
today it is routinely used as part of the hematological treatment algorithm in six Hungarian centers. The procedure
plays a significant role in the treatment of multiple myeloma, Hodgkin’s and non-Hodgkin’s lymphomas. Further-
more, it has an increasing number of indications in the treatment of non-hematological diseases. Although autolo-
gous hematopoietic stem cell transplantation is associated with fewer acute complications than allogeneic stem cell
transplantation, a number of short- and long-term cardiovascular side effects may occur during therapy, mainly as a
result of high-dose chemotherapy pretreatment and potentially cardiotoxic oncological therapies used previously in
connection with the treatment of the underlying disease. The incidence of acute cardiovascular events occurring dur-
ing the peritransplant period and the subsequent 100 days, as well as late (>100 days) cardiovascular events, is sig-
nificantly higher than in the general population of similar age. Major adverse cardiac events occurring during trans-
plantation can limit the success of therapy and worsen both the prognosis of the underlying hematological disease
and the quality of life of the treated patient. Therefore, early recognition and prediction of these events are important
and form the basis of primary and secondary prevention, improving the prognosis and quality of life of patients. The aim of our review article is to summarize the potential cardiovascular side effects of autologous hematopoietic stem
cell transplantation, their monitoring, and appropriate management in the light of current international hematology
and onco-cardiology guidelines.

AB  - Bevezetés: Amyloidosis esetén a szervezetben abnormális szerkezetű fehérjék (amyloidok) képződnek, amelyek lerakódása szervi diszfunkcióhoz vezet. Szisztémás esetben bármely szerv érintett lehet, így az emésztőrendszer is. A leggyakoribb gastrointestinalis tünetek a dysphagia, a jóllakottságérzés, a gastroparesis, a gastrointestinalis dysmotilitas, a vérzés és a székrekedés. Célkitűzés: Célunk volt feldolgozni a Semmelweis Egyetemen gondozott, gastrointestinalis érintettséggel járó amyloidosisos betegek adatait. Módszer: A Semmelweis Egyetemen 2009 és 2024 között amyloidosissal diagnosztizált betegek közül a gastrointestinalis érintettséggel járó esetek adatait gyűjtöttük össze retrospektív, konszekutív módon a kórházi számítógépes nyilvántartó rendszer segítségével. Rögzítettük a demográfiai adatokat, az endoszkópos, a képalkotó és a szövettani eredményeket, illetve a diagnózis szempontjából releváns laboratóriumi paramétereket. Eredmények: Összesen 15 beteg adatait dolgoztuk fel, ebből 12 esetben könnyűlánc-amyloidosis igazolódott, míg 1-1 beteg esetén örökletes formájú transztiretin-amyloidosis, lysozym-amyloidosis és amyloid-A-fehérje-amyloidosis fordult elő. A mediánéletkor a diagnóziskor és a halálozáskor is 65 év volt, ami vélhetőleg a késői stádiumban felállított diagnózist tükrözi, illetve a magas Mayo-score (átlagértéke: 2,67) a könnyűlánc-amyloidosis kedvezőtlen prognózisát vetíti előre. A gastrointestinum mellett a leggyakrabban érintett szervek a szív (73,3%), a vese (33,3%) és a perifériás idegek (40%) voltak. A fő emésztőrendszeri tünetek az akaratlan fogyás (20%), feltehetően malabszorpció miatt, a nyelészavar (6,6%), a haematochesia (6,6%), a hasi fájdalom (6,6%) és a hasmenés (6,6%) voltak. A leggyakoribb laboratóriumi eltérés az anaemia és a krónikus veseelégtelenség, míg a legtöbbször leírt gasztroszkópos eltérés a gastroduodenitis volt, gastroparesis, gyomorfekély és polypoid képletek mellett. Megbeszélés: Az emésztőrendszeri diszfunkció hátterében álló mechanizmusok világos megértése még mindig hiányzik. Az ereket és idegeket károsító amyloidlerakódás következtében alakulhat ki gastroparesis, hasmenés, polyneuropathia, valamint malabszorpció, illetve az endoszkópia során látott haematochesia és okkult vérzés lehetnek a vérszegénység forrásai. Következtetés: A szisztémás amyloidosis számos szervet érintő, rossz prognózisú kórkép. A tünetek, a laborparaméterek és az endoszkópos eltérések változatosak, de nem specifikusak, így a mihamarabbi diagnózishoz elengedhetetlen a társszakmák szoros együttműködése. | Introduction: In case of amyloidosis, an abnormal protein is formed which deposits and leads to dysfunction. Any
organ can be affected. The most common digestive system symptoms are gastrointestinal dysmotility, dysphagia, sa
tiety, gastroparesis, bleeding and constipation.
Objective: Review of amyloidosis patients with gastrointestinal involvement treated at Semmelweis University, taking
symptoms, endoscopic and histological results into account.
Method: 15 amyloidosis patients with gastrointestinal involvement treated between 2009 and 2024 were collected
using the hospital’s computerized registry system. Results: In 12 patients, amyloid light chain amyloidosis was confirmed, while transthyretin amyloidosis variant was
described in 1 case, lysozyme amyloidosis in 1, and amyloid A amyloidosis in 1 patient. The median age at diagnosis
and death was 65 years, reflecting the latestage diagnosis, and the high Mayoscore (mean value: 2.67) predicted a
poor prognosis for light chain amyloidosis. Other affected organs were heart (73.3%), kidneys (33.3%) and periph
eral nerves (40%). The leading gastrointestinal symptoms were unintentional weight loss (20%), dysphagia (6.6%),
hematochezia (6.6%), abdominal pain (6.6%), and diarrhea (6.6%). Common laboratory findings were anemia and
chronic renal failure, while the most frequently described gastroscopic abnormalities were gastroduodenitis, gastro
paresis, ulcers and polypoid formations.
Discussion: A clear understanding of the mechanisms underlying gastrointestinal dysfunction is still lacking. Gastro
paresis, diarrhea, polyneuropathy, and malabsorption can occur as a result of dysfunction resulting from amyloid
deposition that damages blood vessels and nerves. Abnormalities seen during endoscopy may correspond to those
seen in amyloidosis, and ulcers may be the source of hematochezia, occult bleeding, and anemia.
Conclusion: Systemic amyloidosis is a disease with a poor prognosis that can affect many organs. The symptoms,
laboratory parameters and endoscopic findings are varied, but not specific, so close cooperation of the allied profes
sions is essential for the fastest diagnosis.

AB  - Cardiovascular (CV) adverse events (AEs), especially atrial fibrillation (AF) and hypertension, have been reported in patients receiving treatments for chronic lymphocytic leukemia (CLL), including Bruton's tyrosine kinase inhibitors (BTKis). Although these AEs are managed effectively in most cases and AE management guidelines exist, practical management approaches are inconsistent across regions and practices. We aimed to address these inconsistencies by developing consensus recommendations.A European expert panel was assembled comprising eight hematologists and six cardiologists. Literature analysis, expert interviews, and the Delphi method were used to gain consensus on screening, monitoring, and treatment of AF and hypertension statements.Maintaining BTKi treatment is paramount to maximize time to next treatment; for patients at high risk of progression, this can be achieved by appropriately treating hypertension and AF and adjusting the BTKi dose. Patients should be risk-stratified as low, moderate, high, or very high risk of cancer therapy-related CV toxicity and treated according to their disease status so that CLL treatment can be maintained. Patient education on symptom monitoring, home blood pressure monitoring, and electrocardiograms (baseline, every 3 months) are recommended to detect/monitor AF and hypertension. Close collaboration between hematologists and cardiologists is vital to achieve optimal patient outcomes.

AB  - Bevezetés: Primer vagy könnyűlánc-amyloidosis esetében kórosan termelődő monoklonális immunglobulinkönnyűlánc-fragmentumok rakódnak le a szervekben, a kialakuló amyloiddepozitumok pedig diszfunkciót okoznak. Célkitűzés és módszer: Kutatásunk során a Semmelweis Egyetem Belgyógyászati és Hematológiai Klinikáján 2010 és 2024 között könnyűlánc-amyloidosissal diagnosztizált betegek klinikai, illetve túlélési adatait vizsgáltuk, hat időszakra bontva. Különös figyelmet fordítottunk a prognózist lehetségesen befolyásoló tényezők vizsgálatára, így tanulmányoztuk a túlélés kapcsolatát a diagnózis idejével, a betegek életkorával, a citogenetikai háttérrel, továbbá a különböző kezelési sémák szerint. Eredmények: A vizsgált időszakban összesen 56, könnyűlánc-amyloidosisos beteget diagnosztizáltunk. A növekvő esetszámokból megfigyelhető az a tendencia, hogy egyre gyakrabban ismerik fel a betegséget, a legtöbbször viszont továbbra is csak előrehaladott stádiumban születik meg a diagnózis. A teljes csoport mediántúlélése 44 hónap volt, és nem mutatott szignifikáns javulást a vizsgált időszakok során. A prognózist a leginkább a betegek életkora, illetve a diagnóziskor megállapított stádiumbeosztás befolyásolta. A könnyűlánc-amyloidosis terápiája sok változáson ment át az elmúlt 15 évben. Az újabb típusú szerek – mint a daratumumab és a venetoclax – használata mellett gyakrabban alakult ki komplett hematológiai remisszió, illetve szervválaszt is nagyobb arányban figyeltünk meg a korábban alkalmazott kezelésekkel összehasonlítva. Következtetés: Az újabb típusú gyógyszerekre adott kedvezőbb terápiás válasz hatására számíthatunk a jövőben a túlélési statisztikák javulására is, az utóbbit azonban a vizsgálatunkban eddig nem tudtuk igazolni. A korábbi stádiumban felfedezett betegség jelentősen kedvezőbb prognózist eredményez, ezért a korszerű terápia mellett a korai felismerés szerepét sem lehet eléggé hangsúlyozni.
