We aimed to assess the adherence to inhaled corticosteroids+long-acting β2-agonists (ICS+LABA) in users of the MASK-air® app, comparing the adherence to ICS+formoterol (ICS+F) with that to ICS+other LABA. Materials and methods: We analysed complete weeks of MASK-air® data (2015-2022; 27 countries) from patients with self-reported asthma and ICS+LABA use. We compared patients reporting ICS+F versus ICS+other LABA on adherence levels, symptoms and symptom-medication scores. We built regression models to assess whether adherence to ICS+LABA was associated with asthma control or short-acting beta-agonist (SABA) use. Sensitivity analyses were performed considering the weeks with no more than one missing day. Results: In 2598 ICS+LABA users, 621 (23.9%) reported 4824 complete weeks and 866 (33.3%) reported weeks with at most one missing day. Higher adherence (use of medication ≥80% of weekly days) was observed for ICS+other LABA (75.1%) when compared to ICS+F (59.3%), despite both groups displaying similar asthma control and work productivity. The ICS+other LABA group was associated with more days of SABA use than the ICS+F group (median=71.4% versus 57.1% days). Each additional weekly day of ICS+F use was associated with a 4.1% less risk in weekly SABA use (95%CI=-6.5;-1.6%;p=0.001). For ICS+other LABA, the percentage was 8.2 (95%CI=-11.6;-5.0%;p<0.001). Conclusions: In asthma patients adherent to the MASK-air app, adherence to ICS+LABA was high. The effect of olfactory training (OT) on parosmia is still unclear. The primary objective was to investigate the effect of 3 months of OT on COVID-19-related parosmia. The secondary objectives were to study whether improvement in olfactory quantitative function and compliance to OT influenced improvement in parosmia. Methods: This prospective cohort study was conducted from October 5, 2022, to February 7, 2023, at a tertiary smell and taste clinic. Patients with persistent post-COVID-19 parosmia were instructed to perform OT twice daily for 3 months. Improvement in parosmia was measured using self-rated intensity and pleasantness scales, a structured parosmia questionnaire, and the Sniffin' Sticks Parosmia Test (which measures hedonic direction and range). Quantitative olfactory function was measured with Sniffin' Sticks to determine the threshold, discrimination, and identification (TDI) score. Pre- and post-OT assessment of parosmia and quantitative olfactory function was performed. Compliance to OT was calculated as a percentage of the number of OT sessions performed over the expected number of training sessions over 3 months. Results: Thirty-eight patients (mean age 41.8 [+/- 13.3] years, 12 [31.6%] males) were recruited. After 3 months, 52.6% (n = 20) reported perceived improvement in parosmia. There was improvement in perceived pleasantness (-2.0 [-3.0 to -1.0] vs. -3.0 [-4.0 to -2.0], p = 0.027), hedonic range (2.4 [+/- 1.3] vs. 1.9 [+/- 1.6], p = 0.023), and TDI (27.9 [+/- 7.4] vs. 25.2 [+/- 7.2], p = 0.001) from baseline. All measures of parosmia did not differ significantly between patients who achieved the minimum clinically important difference of >= 5.5 in TDI scores versus those who did not. Patients who had >70% compliance to OT displayed more significant improvements in hedonic range (1.1 [+/- 1.1] vs. 0.2 [+/- 1.3] p = 0.04) compared to those who were less compliant. Conclusion: COVID-19-related parosmia and olfactory dysfunction improved with OT over 3 months. Improvement in olfactory function does not necessarily improve parosmia perception. However, compliance with OT appears to have an effect on improvement in parosmia. Biologics are novel therapeutic strategies providing targeted treatment for the difficult-to-treat recalcitrant CRSwNP patients. Dupilumab is a fully human-derived monoclonal antibody that binds to IL4Rα, inhibiting the signalling of both IL-4 and IL-13. In Hungary, it is approved for the treatment of uncontrolled CRSwNP according to criteria based on the EPOS2020 and the Hungarian guidelines. Methodology: This study aimed to collect and evaluate real-world therapeutic data of CRSwNP patients treated with dupilumab. One hundred thirty-five patients from eight different referral centres have been enrolled in this study, who received dupilumab since 2020. All subjects were adult patients (>18 years) with uncontrolled CRSwNP. Baseline data collection included demographics, medical history, previous surgeries, related comorbidities, total endoscopic nasal polyp score (NPS), SNOT22, nasal congestion parameters measured with visual analogue scale (VAS) and nasal obstruction evaluation scale (NOSE), loss of smell score (LSS) and eosinophil count. 300 mg dupilumab was administered subcutaneously every second week. Follow up visits were performed after 6 and 12 months. Results: After 6 and 12 months of treatment significant improvement was detected in all clinical parameters. Safety was proved, no severe side effects occurred, and no rescue treatment was necessary. Conclusions: Our real-life findings show that continuous dupilumab treatment is effective and safe in daily clinical practice in CRSwNP and other type 2 comorbidities such as bronchial asthma and NERD. AB - Background: After a severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) infection, smell disorders frequently occur, significantly affecting patients’ quality of life (QoL). Methods: 110 patients with persistent olfactory disorder after coronavirus infection were enrolled. These patients underwent chemosensory testing using the Sniffin’ Sticks test, and completed the Questionnaire of Olfactory Disorders (QOD). Results: 30% of the patients reported anosmia, and 70% reported hyposmia. Upon comparing subjective and chemosensory testing categories, good category matching was observed in 75.3% (i.e., anosmia based on both methods in 10 and hyposmia in 48 cases). Statistical analysis using the Chi-square test revealed a significant result (p = 0.001 *). Between the TDI (i.e., Threshold, Discrimination, Identification) results of the three subjective report groups (i.e., hyposmia, anosmia, and parosmia), no significant differences were observed. When the TDI and QOD results were compared, no consistent significant correlations were found in most TDI and QOD outcomes. Between the TDI and Scale 2 results, a significant, although slight correlation was observed by the Spearman’s (rho = 0.213, p = 0.027 *) and Pearson’s (rho = 0.201, p = 0.037 *) tests. Conclusions: The nonsignificant correlation between objective and subjective methods suggests that these results should be interpreted independently.
